These Terms of Service ("Terms") govern your use of the Transcend Mind-Body Wellness website, services, and communications, including SMS/text messaging. By using our services, you agree to these Terms.
Transcend Mind-Body Wellness provides psychiatric evaluation, medication management, and therapy services for adults. Our services are provided by board-certified Psychiatric Mental Health Nurse Practitioners in the states of Oregon and Washington.
Our services do not constitute emergency mental health services. If you are in crisis, please call 911 or the 988 Suicide & Crisis Lifeline by dialing 988.
Our services are available to adults (18+) residing in Oregon or Washington. By booking an appointment, you confirm that you meet these eligibility requirements and are located in one of these states at the time of your telehealth appointment.
Appointments are booked through our online scheduling system (ZocDoc) or by calling our office. Cancellation and rescheduling policies are communicated at the time of booking. Late cancellations or no-shows may be subject to a fee.
We accept most major insurance plans including Aetna, Blue Cross Blue Shield, Cigna, United Healthcare, Moda, and First Choice. Self-pay options are also available. Co-pays and deductibles are due at the time of service. You are responsible for verifying your insurance coverage prior to your appointment.
By providing your phone number and consenting to receive text messages from Transcend Mind-Body Wellness, you agree to the following:
Carriers are not liable for delayed or undelivered messages.
Compatible carriers include but are not limited to: AT&T, Verizon, T-Mobile, Sprint, U.S. Cellular, and other major carriers.
Telehealth appointments are conducted via secure, HIPAA-compliant video platforms. You are responsible for ensuring a private, stable internet connection for your appointment. Telehealth services are available only to patients located in Oregon or Washington at the time of their appointment.
We prescribe controlled substances (including stimulant medications for ADHD) when clinically appropriate. All controlled substance prescriptions are managed in compliance with DEA regulations and state prescribing laws for Oregon and Washington. We reserve the right to require in-person follow-ups for controlled substance management.
Transcend Mind-Body Wellness is not responsible for any delays, failures in delivery, or issues related to the transmission or receipt of text messages. Delivery of text messages is subject to effective transmission by your mobile carrier and is not guaranteed.
While we strive to provide the highest quality psychiatric care, individual treatment outcomes may vary. Our services are not a substitute for emergency medical care.
All content on our website, including text, images, and design, is the property of Transcend Mind-Body Wellness and is protected by copyright law. You may not reproduce, distribute, or modify any content without our written permission.
Your use of our services is also governed by our Privacy Policy, which describes how we collect, use, and protect your personal information.
We may update these Terms from time to time. Changes will be posted on this page with an updated date. Continued use of our services after changes constitutes acceptance of the updated Terms.
